The goal of the Tobacco-Free Recovery (TFR) Initiative is to reduce commercial tobacco use among individuals with behavioral health conditions, including those with mental health and/or substance use conditions. To ensure consistency in benchmarking and tracking progress across participating states and territories, the initiative utilizes five Behavioral Risk Factor Surveillance System (BRFSS) measures, along with select indicators from SAMHSA’s National Substance Use and Mental Health Services Survey (N-SUMHSS), to monitor outcomes over time.
This state page also provides a high-level overview of each state’s Tobacco-Free Recovery action plan strategies, illustrating the approaches being implemented to advance tobacco-free recovery goals.

Tobacco use by population
CDC Behavioral Risk Factor Surveillance System (BRFSS), is consistently collected on an annual basis and is comparable because it uses the same metric definitions across all states. BRFSS data is collected and available for all 50 states, as well as District of Columbia and 3 US territories with more than 400,000 adult interviews annually.
These five self-reported measures from BRFSS enable the nation and states to monitor trends in prevalence of smoking for people with behavioral health conditions. These include adult smoking prevalence, 2 mental health-based metrics (current smoking among adults with frequent poor mental health and current smoking among adults with depressive disorder) and 2 substance use-based metrics (current smoking among adults who drink heavily and current smoking among adults who binge drink). The table describes most recently available data for each of the five measures.

SAMHSA National Substance Use and Mental Health Services Survey (N-SUMHSS) collects data from SAMHSA funded substance use facilities across the nation to assess at the national, state, and substate level, the extent to which the behavioral health system offers services that promote access to tobacco use treatment services. The table describes tobacco interventions in these substance use health facilities nationally and reflects most recently available data.
